There are increasing and evolving health challenges across the population from young to old. This includes the increased prevalence of comorbid conditions and changes in health care systems in both the developing and developed world. This has intensified the focus on clinical complexity arising from medical, psychological, social, and behavioural factors. At Psychology Cares, we have placed an increased focus on a care model which is designed to tackle the multifaceted challenges faced by complex patients. Comprehensive assessments and integrated care plans are pivotal in addressing these challenges effectively.
At Psychology Cares, we believe effectiveness of care is determined by how it meets your needs across a matrix of health and wellbeing outcomes. The World Health Organisation (WHO) also recognises the importance of integrated health care in its vision and strategy for global health service delivery.
According to WHO, integrated care is “an approach to strengthen people-centred health systems through the promotion of the comprehensive delivery of quality services across the life-course, designed according to the multidimensional needs of the population and the individual and delivered by a coordinated multidisciplinary team of providers working across settings and levels of care[…]the appropriate use of resources based on the best available evidence, with feedback loops to continuously improve performance and to tackle upstream causes of ill health and to promote well-being through intersectoral and multisectoral actions”

Insufficient collaboration among healthcare providers poses significant challenges to clinical effectiveness. Mental and physical health service users often encounter fragmented services, fostering feelings of insecurity, frustration and ultimately despair. This lack of coordination further exacerbates the inadequacy of services, prolonging illness, disease and ill health. Ultimately, curtailing recovery.
Integrated care for many service providers remains a complex phenomenon. It necessitates integrating understanding and skills at multiple levels, requiring variation and adaptation and innovation in interventions across a number of stakeholders, contextual factors that influence and impact patient care and treatment outcomes. Therefore, specialist care does not always mean individualised or integrated care.
In view of our commitment to integrated health care service delivery that puts individuals complete health at the centre of our care programmes and treatment approaches, Psychology Cares has developed bespoke healthcare services to meet your needs at all stages of life. We believe that we can promote excellence in care to meet the physical and psychological health needs of all.

Not understanding the correct sequence & contribution (differential diagnosis) of a health problem can adversely impact patients’ physical & mental health.
People continue to experience significant gaps in the availability of healthcare services, with at least ½ of the population unable to obtain such essential services.
Connected care, allowing for the provision of coordinated medical services, enhances quality of care & treatment outcomes.
